What is a Home Care Package?

The Australian Government’s new aged care system, the Support at Home Program, will replace the Home Care Packages Program from 1 July 2025.
Anchored Care partners with Trilogy Care, one of Australia’s leading Support at Home Program providers, to make it easier for you to access the care and services you need at home.

What is the Support at Home Program?

The Support at Home Program is the upcoming national aged care system that combines existing programs such as Home Care Packages, the Commonwealth Home Support Programme, and Short-Term Restorative Care into a single, streamlined model. It is designed to make it easier for older Australians to receive coordinated, flexible care at home.
Under the Support at Home Program, your funding will go directly to an approved provider (such as Trilogy Care) to coordinate your care services.
You will be able to choose the services that best meet your needs, including:
  • Personal care (showering, dressing, mobility assistance)
  • Domestic assistance (cleaning, laundry, gardening)
  • Nursing and allied health
  • Meal preparation
  • Social support and transport

  • Increase your care hours with lower management fees
