We offer this service across South East Queensland, New South Wales, and South Australia.
How it works
- Our experienced nurses and trained support staff provide personalised care that promotes comfort, wellbeing, and independence.
- We collaborate closely with allied health professionals, GPs, and community teams to make sure your supports are consistent and coordinated.
Psychosocial Support
Our team provides recovery-focused and trauma-informed care for people with psychosocial disabilities. We focus on stability, confidence, and community connection.
Supports may include:
- Recovery coaching and goal setting
- Assistance with medication and health monitoring
- Emotional regulation and behavioural support strategies
- Encouragement to engage with therapy, education, or work
- Collaboration with allied health and community mental health teams
Physical Support
For people with complex physical or health-related needs, we provide safe and clinically guided care that promotes independence at home and in the community.
Supports may include:
- Medication administration and clinical monitoring
- Manual handling, transfers, and mobility support
- Wound care and continence management
- Mealtime assistance and PEG feeding
- Collaboration with GPs and allied health professionals
